STAGE I: Patient Qualification for Admittance to the Diagnosis and Treatment Department

To be referred to the hospital, a patient must first attend an eligibility consultation with a specialist, during which the consulting physician and the patient agree on the necessary diagnostic and therapeutic procedures. Next, a convenient procedure date is scheduled.
Once qualified for surgery or diagnostic procedures, the patient receives a document specifying the date and time of admittance to the hospital and the necessary preparatory measures.

If the attending physician refers the patient for a consultation with an anesthesiologist, the appointment can be arranged at the Hospital’s Reception Desk. The patient can register in person, by phone (12 379 73 90), by email (at or via the e-registration form available at

STAGE II: Admittance to the Hospital

Patients are asked to arrive at the Department’s Registration Desk (Building B, 1st floor, Elevator 3, room 7.6).
Please make sure to bring the following items with you:

  • ID card
  • the results of any diagnostic tests that were ordered
  • vaccination certificate (type B hepatitis)
  • toiletries, towel, pajamas or nightgown, bathrobe, and slippers
  • your medication (if any)

Stage III: Discharge from the Hospital
The date of discharge is specified by the attending physician.
On the scheduled date of discharge, the patient will receive a Hospital Discharge Summary Report featuring information about the hospitalization and recommendations for further treatment, as well as a medical certificate of incapacity for work, if applicable.

To get a certificate of incapacity for work, the patient must provide the tax identification number (NIP) of his/her place of employment. The patient should keep the Hospital Discharge Summary Report and bring it to follow-up appointments.
Post-surgery (post-hospitalization) follow-up appointments take place at the Specialist Outpatient Care Department on the appointed dates.
